What are the eligibility requirements?
• Be 25 years of age or older.
• Have never been convicted of a sex crime (other than prostitution). • Be under supervision at time of enrollment at Newton Correctional Facility, Iowa Correctional Institute for Women, Fort Des Moines Community Corrections or Fresh Start Women’s Center. • Be low-income. (Participants residing in target area are considered to be low-income.) • Be planning to return to live in the counties of Polk, Dallas or Warren (target area) after release. • Be on track to be released within 6 months from the start of training. • Male participants must be registered for selective service or initiate the process to determine if failure to register was knowing and willful. |
What is an incentive payment?
This is only earned if a participant completes their cohort and obtains at least 2 of the offered certificates.

What is the supportive services limit for each participant?
It can be less, but without special permission, it cannot be more than $400 total.

What is Credit Card Approval New GL?
This form is used to request a credit card for things like gas cards, cell phones and clothing.

What is a resume?
By sitting down and completing this document with a participant, case managers can often help increase the wage of the job a participant receives.

What is a signed attendance sheet?
One of these must be turned in before United Way will pay any partner's invoice.

What is cell phone assistance?
If a participant has trouble receiving phone calls from potential employers, this may be able to help.

What is the job development log?
This form allows a case manager to keep track of the places they have applied with a participant as well as any usernames or passwords that were used.

What are job applications?
ZipJob suggests that a job candidate should be filling out 10-15 of these per business week. CIRCA case managers do at least 3 of these per visit when a participant is looking for work.

What are the contact expectations?
At least bi-weekly from the enrollment date until 12 months after release.

What are documents required upon release?
These corroborate name, birth date and address. All of this is necessary to prove to the DOL that a participant qualifies for the funding.

What is a completed application form, the bus pass number and a signature?
These 3 things are needed to prove to DART that an OTT pass was given to a qualified individual.

What is the first meeting checklist?
This form helps a case manager to check off all the things they need the first time they see a participant after release.

What is a certificate?
Participants complete up to 4 of these in a cohort and should be able to request a copy from their case manager at any time if they were to lose the original.

What is a signed release of information?
To be able to speak to anyone about a specific participant.

What is the legal assistance that Legal Aid can provide before needing permission from CIRCA?
Up to 2 hours to resolve legal issues.

What is a W-9?
This form is needed any time a participant will be receiving over $600 of assistance from a United Way program.

What is the Post Release Follow Up Form?
This form allows case managers to visibly see how often they are in contact with their participants after release.

What is STAR?
This method teaches a participant to make sure to be sharing the results of their accomplishments during an interview.

What is a high school equivalency?
Not necessarily required for the retail/hospitality cohort, this is required for CNA, construction and TDL.

What is 3 years?
All records must be kept for this many years in case the DOL would like to review them.

What is $1,000?
Central Iowa Works may use general funds (not CIRCA funds) to assist with driver's license fines and fees up to this dollar amount.

What is the Fund Disbursement Form?
This form is completed each time a case manager wants to provide financial assistance to a participant.

What is a disallowed cost?
If someone is found ineligible for any reason, including lack of documentation, they are now considered this for our funding.
