What are the 5 phases for the Elementary application process?

1.The Educational Coordinator, Program Director, Family Support Specialist, Mental Health/Disabilities Specialist, and Family Support Coordinator will meet to discuss an appropriate action plan for the child. The action plan may include referrals to the mental health consultant, Special Education Coach, and/or other behavior professionals for observations/evaluations.

2.Within two weeks the staff that designed the child’s action plan will meet with parent/guardian to discuss the plan and work together to prevent the behavior(s) in the future so that the child may achieve success or improvement. At this time, the parent/guardian may be asked to consult with EHDN staff weekly or as deemed necessary to monitor and review the child’s progress.
