You observe caregiver being very responsive to baby cues, speaking softly to baby, smiling at baby and gently rubbing babies back

Caregiver had GDM, blood sugars normal since birth.

No complications, had all tests completed, immunizations UTD

Neither caregiver nor partner have a primary care provider

The childcare plan caregiver had in place has fallen through. Caregiver is unable to afford childcare for her baby and 3 yo child

Caregiver feels guilty because she does not enjoy breastfeeding and wants to stop breastfeeding

Caregiver plans to get an IUD which she has had in the past.

Both caregiver and baby have private insurance, caregiver has upcoming appointments for the baby and herself scheduled

Caregiver is undecided on when and how much she will be working in the future. She is interested in local nanny services

On Exam: Alert, appropriate, caregiver state infant has tiny bumps on the face that is consistent with normal normal newborn acne

Caregiver is uncertain if baby has been added to her Mediciad

Caregiver has daily care (local day care); and emergency care. When asking about respite care she said "yeah- my mother in-law offers, but I don't trust her with the baby"

Caregiver states that she does not believe you can spoil a baby and is responsive to baby when crying even though her family tells her “not to hold the baby all the time”

Caregiver has a 2nd degree laceration which is very tender and "seems to be getting worse each day". Low grade fever but "doesn't feel sick"

Baby birth wt of 8 lb 3 oz. Baby's wt at the 3 week IHV is 8 lb 1 oz.

While in an IHV the nurse calls the OB office to schedule a postpartum appointment

Caregiver and partner will both be returning to work at 6 weeks. They have childcare through caregiver’s employer and lots of local family and friends to provide respite care. No emergency care is in place, but caregiver will talk to her neighbor to create an emergency plan.

Caregiver states this was an unexpected pregnancy and she was very concerned about caring for another child, but since the baby’s birth she can’t imagine life without this baby
