Caregiver describes baby as "very chill" and she only cries when she is hungry

Family needs an intake appointment with WIC

Both caregiver and partner report hearing gunshots in their neighborhood every few days. To feel safe, they do not go out at night.

Caregiver does not have any history of CPS cases and childhood was very happy and supportive

RN provides general information regarding baby blues, review the s/sx of PMD and the list of community support from your resource folder in case they need it in the future and you remind caregiver and partner that perinatal mood disorder can occur up to a year postpartum.

RN demonstrates the square swaddle and reviews the 5 S's

Although there is a basinet in the home, baby is bed-sharing. Caregiver understands the safety risk and is willing to try swaddling and other techniques to help baby sleep independently.

Caregiver has a restraining order against the father of the baby and needs information on legal custody issues

Caregiver reports that her own father was an alcoholic and verbally abusive. She no longer has any contact with him.

Caregiver states she did experience PPMD with her first child, but did not seek any treatment. She is currently feeling better than she expected and is relieved that she feels very different in this postpartum period.

Caregiver states that she can become "annoyed" with baby's crying. Discuss options and will ask for help from her partner when this happens.

Family lives in a new home and has all needed baby care items

Caregiver and partner are both present for IHV. You observe very respectful, supportive interactions between the couple. The screener indicates no IPV concerns

There is a CPS case currently open, baby tested positive for THC at birth

Caregiver is bipolar. She has medication management, but is not being followed by a provider therapist.

Caregiver states the baby stops crying as soon as she picks him up

Since caregiver is unable to work until she gets medical clearance at her postpartum appointment, she is having difficulty paying utility bills

Screener for IPV is negative, but RN is unable to verbally question or observe caregiver about IPV

Caregiver reports no hx of abuse or neglect, but would like to parent differently than she was raised - she doesn't want to spank or yell.
