When asked to describe her baby, caregiver states “she is spoiled” and “she wants to be held all the time”

The home is older, built in 1969 and caregiver is uncertain if it has been assessed for lead.

Baby is on a waitlist for daycare

Caregiver has smoked cigarettes through pregnancy and is continuing to smoke in the postpartum period when she is “stressed out”. She is interested in trying to quit

Caregiver is new to the area. She states she feels supported by her partner and sister but wishes she had more friends.

Breastfeeding is a struggle d/t painful latch

Baby is being fed every hour and caregiver states it is the only way she can get the baby to stop crying

Since caregiver is not working until she gets medical clearance at her postpartum appt, she is concerned about paying utility bills

Partner does not live with family but is over to help out mom whenever needed. Mom reports that he smokes marijuana, but not in the house.

This is the families 4th child. Breastfeeding is going great and there is a large church community providing good emotional and childcare support

Caregiver has Medicaid for herself and the baby, but is not yet registered with WIC

Although there are no current CPS cases, there is a history of a closed CPS 5 years ago.

Partner is a smoker, but only smoking in the garage.

Caregiver has a hx of drug addiction, but has been sober for 3 years. She is currently going to NA meetings weekly, has a therapist and good emotional and childcare supports in place

Twins have arrived for first time parents. This family is very worried about how they will manage once their parents are no longer able to stay in the home to help them.

Caregiver does not yet have her postpartum appt scheduled and would like the nurse to schedule this

Partner is concerned about sibling regressions since the birth of baby

Baby continues very sleepy and her weight gain is below expectations
