A consumer who is abused or neglected has the right to pursue _ _ _ _ _ _ _ _ _ _ and other appropriate civil relief.

Incidents where physical _ _ _ _ _ _ _ _ _ _ _ _ is used will be documented on an Incident Report and sent to the SCCMHA ORR.

Medication that is given to recipients shall be in _ _ _ _ _ _ _ _ _ _ with state rules and federal regulations pertaining to labeling and packaging.

Neither SCCMHA staff members or members of the SCCMHA Service Provider Network shall make _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ regarding sterilization or abortion.

Consent for the use of fingerprints, photographs, audiotapes, or one-way glass may be _ _ _ _ _ _ _ _ _ at any time.

As stated in the Application Section of this policy, this policy applies to all SCCMHA direct _ _ _ _ _ _ _ _ programs as well as all SCCMHA Service Provider Network programs.

SCCMHA _ _ _ _ _ _ _ _ the use of non-restrictive techniques to address challenging behaviors.

Medication shall be prepared and _ _ _ _ _ _ _ _ _ _ _ by qualified and trained staff.

The consumer, their guardian, parent of a minor or loco parentis will be directed to the _ _ _ _ _ _ _ _ _ _ _ county or private agency available to provide more information.

All consumer consents related to fingerprints, photographs, audiotapes, one-way glass, or written information for SCCMHA publications will be _ _ _ _ _ _ _ _ _ by using the Regional Release form in sentri II.

The worth, dignity and _ _ _ _ _ _ _ _ _ _ of all consumers, as well as their rights and opportunities, shall be respected and promoted.

Consumers in need of a Behavior Plan due to challenging behaviors will be _ _ _ _ _ _ _ _ to a Behavioral Psychologist for a comprehensive assessment/analysis

Administration of medications shall be _ _ _ _ _ _ _ _ in the consumer’s clinical record.

The Support Staff responsible for the consumer’s written plan of service shall provide _ _ _ _ _ _ to the consumer, their guardian, parent of a minor or loco parentis of the availability of family planning and health information.

The _ _ _ _ _ _ _ _ _ _ _ of fingerprints, photographs, or audiotapes is described in Procedures #3, 4, & 5 below.

A consumer of SCCMHA shall not be _ _ _ _ _ _ _ _ _ to abuse or neglect.

Physical management shall not be included as a _ _ _ _ _ _ _ _ _ of Behavior Treatment Plans.

Medication errors and _ _ _ _ _ _ drug reactions are immediately reported to the RN or physician, and documented in the clinical record.

The Support Staff responsible for the consumer’s written plan of service shall provide referral _ _ _ _ _ _ _ _ _ _ to providers of family planning and health information services upon request of the consumer, their guardian, parent of a minor, loco parentis.

Fingerprints, photographs, or audiotapes may be taken and one-way glass may be used in order to _ _ _ _ _ _ _ _ _ the identification of the consumer as set forth in Procedure #4 below.

Any _ _ _ _ _ _ _ _ _ _ _ who makes a report under Section 330.1722 of the Michigan Mental Health Code shall not be dismissed or otherwise penalized by an employer or contractor for making the report.

Prone Immobilization is _ _ _ _ _ _ _ _ _ _ unless other techniques are medically contraindicated and documented in the record.

Medication use shall _ _ _ _ _ _ _ to standards of the medical community.

Notice shall be given to the consumer, their guardian, parent of a minor or loco parentis, _ _ _ _ _ _ _ _ _ _ that the receipt of mental health services, release, or discharge, is in no way dependent upon the request or decision to act on the family planning information.

In the event that fingerprints, photographs, or audiotapes are taken in order to provide services to a consumer, all _ _ _ _ _ _ of them shall be kept as part of the record of the consumer.
